What To Expect On My First Visit?
You will be scheduled for a one hour, face to face, consultation with Dr. Shulman.
This time is reserved just for you so we ask that any changes in appointments be made at least 24 hours prior to your scheduled appointment. At the first visit Dr. Shulman will go over all of your medical history and any previous sleep studies that have been performed. If you have not had a sleep study Dr. Shulman can prescribe one for you. You will receive a detailed description about sleep apnea and exactly how an oral appliance can help. A full oral examination of your mouth and teeth will be done to insure that there is good oral health to support an oral appliance.
What to bring to your appointment.
- All Medical and Dental Insurance cards and a picture ID.
- A complete list of all Medications you are taking. (Please note that your new patient forms can be filled out this website under Sleep Apnea Patient Registration ).
- Any referrals or prescriptions to see Dr. Shulman.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Please notify the office if you have a medical condition or concern prior to treatment (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
We will complete an in-depth medical health history and a thorough dental examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease that may compromise your treatment.
